How diabetes care is changing in Cornwall

Diabetes care in Cornwall takes an important stride forward

People living with diabetes across Cornwall are being encouraged, involved and supported to take vital steps towards better long-term health with a variety of support available.

Leading the way and providing a varied package of support to patients and their families are the team from Three Harbours and Bosvena Primary Care Network (PCN), who have been working with NHS Cornwall and Isles of Scilly to:

  • deliver local ‘diabetes super clinics’ to enable people with diabetes to receive their 8 separate annual diabetes checks in one single visit
  • set up a number of diabetes support groups tailored for people with type 2 diabetes and those with type 1 diabetes
  • support a local diabetes health walk at the National Trust’s Lanhydrock estate near Bodmin, as part of the Cornwall Active Travel Social Prescribing pilot, which is delivered by Cornwall Council.
