There is a new self-referral form patients can complete for a new pregnancy referral to the midwives. You can either scan a QR code on a poster in the surgery, call 01209 881788 or we can send you a link to the form to complete.
The maternity department are trying to encourage you to use this self-referral form as they get access to your maternity records electronically, it completes a risk assessment on the patient for how quickly the midwife should get in contact and gives some advice on when the midwife will be in touch.
New pregnancy patients can self refer to the midwives using this form.
